Primary Care and Occupational Health:
Monday – Friday, 8 a.m. – 5:30 p.m.
Saturday, 8 a.m. – 12 p.m.
Monday – Friday by appointment with same-day appointments accommodated. Open Saturday for walk-in

+ Who is eligible to use The Coalition Family Health Center?
The Coalition Family Health Center is available to those enrolled in sponsored insurance plans of Navajo Transitional Energy Company, Eagle Specialty Materials, and Peabody Energy. The health center is a convenient, full-service healthcare facility for the use of employees, retirees, and dependents who are enrolled in a participating company’s insurance plan.

+ How do I schedule an appointment?
You can schedule an appointment by calling 307-685-6500 or visiting mypremisehealth.com. If you are experiencing a medical emergency please call 9-1-1.
+ Are appointments required or can I walk in?
The Coalition Family Health Center is available Monday – Friday by appointment only, with same-day appointments accommodated. The health center is open Saturday for walk-in and urgent care only.
+ What services does the health center provide?
You and your family have convenient access to:
Primary Care Health Services:
- Annual physicals
- Lipid profiles, blood glucose, and blood pressure screenings
- Allergy management
- Assess, evaluate, and treat patients
- Administer medications to patients
- Prescribe and monitor ongoing medications
Preventive Services:
- Flu shots, immunizations, and vaccinations
- Men’s and women’s health
- Skin, breast, and prostate cancer screenings
- Travel health consultations
Acute/Urgent Care:
- Common cold; flu symptoms
- Ear aches; sinus infections
- Sprains and strains
- Minor surgical procedures
- Community acquired pneumonia
- Pharyngitis
Clinical Lab Services:
- Provide on-site laboratory specimen collection and lab work
- Follow up lab monitoring for on-site providers and off-site community providers
Disease/Condition Management:
- Diabetes, high cholesterol, and hypertension
- Asthma, COPD
- Back pain, migraine, and osteoarthritis
- Depression, stress, obesity/overweight and smoking cessation

Additionally, The Coalition Family Health Center works closely with area specialists, area hospitals, and other facilities for more complex health issues, surgeries, and emergency care.
+ What medical plans are accepted?
The Coalition Family Health Center accepts the sponsored insurance plans of Navajo Transitional Energy Company, Eagle Specialty Materials, and Peabody Energy.
+ If I do not have insurance through Navajo Transitional Energy Company, Eagle Specialty Materials, and Peabody Energy, can I still use the health center?
No. You must be covered under one of the participating company's sponsored insurance plans to use the health center.
+ Can I do lab/blood test orders from another provider at the health center?
Yes, the health center can perform blood draws ordered by an outside provider. We request all lab orders to be faxed or dropped off at our office to ensure we have proper supplies to run the requested testing.
+ Is the health center an approved provider for workers’ compensation claims? If I get hurt on the job, can/should I go to the health center?
Workers’ compensation claims are only approved for Navajo Transitional Energy Company, Eagle Specialty Materials, and Peabody Energy .
+ Who manages The Coaltion Family Health Center?
Premise Health operates the health center.
+ Is my health data secure and private? Will my employer have access to my medical records?
Your privacy is of the utmost importance. Your personal health information is protected in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy and Security Rules.
Providers working in the health center have access to your medical records. Select team members of the health center also have access to certain information as part of the normal data and claims process.
Due to the sensitive nature of this information, team members with access to health center data have undergone additional privacy training and are required to attest to confidentially requirements. They are only authorized to access the minimum information necessary to perform their job duties. Access to health center patient information is audited and any unauthorized access is grounds for disciplinary action up to and including termination.
+ Am I required to use the health center?
No, the health center is offered as a convenience for eligible employees, retirees, and dependents.
+ Will I have to change my doctor if I use the health center?
No. The health center is offered as a convenient benefit to Navajo Transitional Energy Company, Eagle Specialty Materials, and Peabody Energy employees, retirees, and dependents. You are not required to discontinue services with any of your current/future outside providers.
